Accepting a request for special testing and studying accommodations
Our university provides testing and studying accommodations for applicants with disabilities or special needs to prevent them from being forced to take exams or study under disadvantageous conditions. If you wish to request testing accommodations, please consult with us prior to submitting an application for admission in accordance with the following request procedure.
- Please make sure that you consult with us even if you are planning to use at the time of exam a hearing aid, crutch, wheelchair and other items you normally use, as accommodations may still be needed in these cases.
How to submit your request
Please submit the documents listed in (i) to (iv) (please submit (iii) and (iv), only if you can) below by the deadline specified in our Application Guidelines. We will review your request based on the documents you submitted to provide proper accommodations. (We may ask questions to you, your guardians or other relevant persons in our review process.)
(i) Request form (PDF)
Download the request form (PDF) above and fill out the form.
(ii) Doctor’s certificate (PDF)
Download the doctor’s certificate form (PDF) above and ask your doctor to fill out the form. Please request your doctor to write the specific level of your disability and the reason why testing accommodations requested by you are needed.
* If you have requested accommodations at the time of National Center Test for University Admissions, notification under
(iii) and a copy of doctor’s certificate you submitted to the National Center for University Entrance Examinations are sufficient. You do not need to submit a doctor’s certificate in a form specified by our university.
(iii) Notice of decision on testing accommodations for National Center Test for University Admissions
If your request for testing accommodations for National Center Test for University Admissions has been accepted, please submit a copy of “notice of decision on testing accommodation” sent by the National Center for University Entrance Examinations.
(iv) A copy of physical disability certificate
Please submit a physical disability certificate, if you have one.
Request deadline
Please check the request deadline specified in our Application Guidelines. If testing accommodations become necessary after the request deadline, please contact us immediately for consultation. Please submit your request as early as possible, as it might take time to review your request depending on the type and level of disability.
